Early relapse under the LoVAS reduced-dose glucocorticoid regimen in ANCA-associated vasculitis: a single-center retrospective observational study.

To evaluate early relapse and other clinical outcomes in patients with microscopic polyangiitis (MPA) or granulomatosis with polyangiitis (GPA) treated with reduced-dose glucocorticoid (GC) regimens, with a focus on the LoVAS regimen in comparison with the PEXIVAS regimen. We conducted a retrospective single-center cohort study of consecutive patients with newly diagnosed or relapsing MPA or GPA treated between March 2022 and September 2025. Patients were classified into LoVAS or PEXIVAS groups if they adhered to the assigned reduced-dose GC regimens for at least 14 days. The primary outcome w

Low-dose rituximab for MPO-ANCA-associated hypertrophic pachymeningitis with delayed onset of renal involvement: a case report.

Hypertrophic pachymeningitis (HP) is a rare disorder characterized by focal or diffuse inflammatory thickening of the dura mater. Patients may present with prolonged localized central nervous system involvement and often lack the typical manifestations of ANCA-associated vasculitis (AAV), making diagnosis and treatment challenging. We report the case of a 55-year-old woman who presented with recurrent headache, right eye pain, and visual decline. Laboratory testing showed positivity for myeloperoxidase anti-neutrophil cytoplasmic antibody (MPO-ANCA), and brain magnetic resonance imaging (MRI)

Myeloperoxidase Antineutrophil Cytoplasmic Antibody (MPO-ANCA)-Associated Usual Interstitial Pneumonia: Pathogenic Mechanisms, Diagnostic Challenges, and Clinical Implications.

Usual interstitial pneumonia (UIP) has traditionally been regarded as synonymous with idiopathic pulmonary fibrosis (IPF) in the absence of an identifiable cause. However, emerging evidence demonstrates that a subset of patients with a UIP pattern exhibit myeloperoxidase antineutrophil cytoplasmic antibodies (MPO-ANCAs), suggesting a distinct and evolving clinical phenotype at the intersection of fibrotic lung disease and systemic autoimmunity. This structured narrative review aims to synthesize current evidence regarding the clinical, diagnostic, and therapeutic implications of MPO-ANCA-assoc

An update on the pharmacotherapy of ANCA-associated vasculitis.

INTRODUCTION: Antineutrophil cytoplasmic antibody (ANCA)-associated vasculitides (AAV) are rare, life-threatening autoimmune conditions requiring complex pharmacological management. Contemporary strategies have transitioned from empirical cytotoxic therapy toward mechanistically targeted approaches, reflecting a deeper understanding of B-cell immunity and the alternative complement pathway. This review synthesizes current guidance and presents recent evidence on treatment modalities and emerging options. AREAS COVERED: This paper reviews and synthesizes current international guidance from the

Large-scale proteome profiling identifies circulating biomarkers for disease activity and organ involvement in ANCA-associated vasculitides.

OBJECTIVE: Anti-neutrophil cytoplasmic antibody (ANCA)-associated vasculitides (AAV) are chronic, relapsing inflammatory diseases, yet reliable biomarkers for detecting relapse and organ involvement remain limited. This study aimed to identify plasma protein biomarkers that distinguish active disease from remission and to explore markers associated with lung and kidney involvement. METHODS: Plasma samples from 113 patients with granulomatosis with polyangiitis or microscopic polyangiitis (68 active disease and 45 remission) were profiled using a proximity extension assay targeting 181 inflamm

Intensive Care Management of ANCA-associated Vasculitides: a Narrative Review.

PURPOSE OF REVIEW: Anti-neutrophil cytoplasmic antibody (ANCA)-associated vasculitides, particularly granulomatosis with polyangiitis and microscopic polyangiitis, are rare but severe systemic diseases frequently requiring intensive care unit (ICU) admission due to life-threatening renal and pulmonary involvement. Despite advances in immunosuppressive therapy, these patients remain at high risk of mortality and end-stage kidney disease, with prognosis driven by organ failure (notably diffuse alveolar haemorrhage [DAH]) and treatment-related complications such as infection. ICU management is co

Changing incidence and serotype trends in ANCA-associated vasculitis: a 30-year population-based study in Norfolk, UK.

OBJECTIVES: The ANCA associated vasculitides (AAV) are rare diseases with poor outcomes if untreated. Over a 30-year period we have investigated the changing incidence of AAV and its subtypes-granulomatosis with polyangiitis (GPA), microscopic polyangiitis (MPA), eosinophilic GPA (EGPA), proteinase 3 AAV (PR3 AAV), and myeloperoxidase AAV (MPO AAV). METHODS: The study population included adults in the county of Norfolk, UK between 1991 and 2020. Diagnosis of AAV was made clinically and supported by EMA classification. Classification by ANCA subtype was possible from 2001. The overall incidenc

Hypogammaglobulinaemia in patients with ANCA-associated vasculitis treated with rituximab.

OBJECTIVE: This study aimed to characterize the occurrence of hypogammaglobulinaemia (HG) in rituximab (RTX)-treated patients with ANCA-associated vasculitis (AAV) in a population-based Swedish cohort. METHODS: Patients with incident AAV (1997-2019) in southern Sweden who received RTX for induction or maintenance therapy were included in this study. Medical records were reviewed to assess the incidence of HG (defined as total IgG < 6.7 g/L; mild 5-6.7, moderate 3-4.9, severe <3), as well as demographics, serology, and treatment history. Incidence rates and predictors of HG were analyzed. Foll

Mechanisms, clinical manifestations and management of cardiovascular diseases in ANCA-associated vasculitis.

ANCA-associated vasculitides (AAVs) are rare diseases characterized by small-vessel necrotizing vasculitis, multiorgan involvement and positivity for ANCAs. The main phenotypes are granulomatosis with polyangiitis, microscopic polyangiitis and eosinophilic granulomatosis with polyangiitis, representing distinct yet partially overlapping entities in terms of pathogenesis, clinical expression and therapeutic management. Patients with AAV face a substantial cardiovascular (CV) and thrombotic risk, with higher rates of myocardial infarction, ischaemic stroke and venous thromboembolism than the gen

Extracellular vesicles exposing complement split products are associated with kidney involvement in antineutrophil cytoplasmic antibody (ANCA)-associated vasculitis (AAV).

BACKGROUND: The complement system is crucial in antineutrophil cytoplasmic antibody-associated vasculitis (AAV) pathogenesis. Extracellular vesicles (EVs) serve as carriers of bioactive substances, influencing the immune system. We aimed to investigate myeloperoxidase-positive EVs (MPO+EVs) exposing complement components (C3a, C4d), terminal complement complex (TCC) and complement factor B (CFB) in relation to disease activity and kidney involvement. METHODS: Plasma MPO+EVs carrying complement products, C3a, C4d, TCC or CFB, were analysed by flow cytometry. Clinical data, including kidney bio

Microscopic Polyangiitis With Temporal Artery Involvement Mimicking Giant Cell Arteritis.

A 63-year-old woman presented with a four-month history of bilateral hearing loss and three months of progressive, painful peripheral neuropathy. Her clinical course was complicated by deep vein thrombosis with pulmonary emboli and an incomplete response to initial empiric glucocorticoid therapy. Following glucocorticoid taper, she developed temporal headaches, jaw claudication, and scalp tenderness, raising concern for giant cell arteritis (GCA). Laboratory evaluation revealed markedly elevated inflammatory markers (erythrocyte sedimentation rate and C-reactive protein), microcytic anemia, mi

Atypical Presentation of Microscopic Polyangiitis with Pericarditis Manifesting as Right-Sided Heart Failure.

Microscopic polyangiitis (MPA) is an ANCA-associated vasculitis that affects multiple organs. However, cardiac involvement is rare in these patients. We herein report a rare case of MPA presenting with marked peripheral edema and weight gain due to acute pericarditis with borderline tamponade physiology and possible effusive-constrictive features, resulting in right-sided heart failure. Myeloperoxidase-ANCA was strongly positive, and the patient subsequently developed rapidly progressive renal dysfunction. A renal biopsy revealed pauci-immune crescentic glomerulonephritis, confirming the diagn

Developing a prediction model for poor prognosis in MPA patients using initial admission examination results: a machine learning study from Southwest China.

BACKGROUND: Microscopic polyangiitis (MPA) is one of the main types of ANCA-associated vasculitis (AAV), but current admission examination indicators are limited in predicting poor prognosis for MPA. This study aims to develop a prediction model for adverse prognosis in MPA patients using initial admission examination results. METHODS: We performed machine learning (ML) algorithms on initial admission examination data to predict adverse outcomes in MPA, such as in-hospital death or self-discharge due to critical condition. We analyzed data from 12,497 patients who underwent ANCA tests in Deya

Advances in the Diagnosis, Treatment and Prognosis of ANCA-Associated Glomerulonephritis.

ANCA-associated vasculitis (AAV) with kidney involvement represents small-vessel vasculitis, characterized by rapidly progressive glomerulonephritis and a high risk of end-stage kidney disease (ESKD) and increased mortality. AAV typically presents with multisystem involvement, with renal manifestations occurring more frequently in microscopic polyangiitis (MPA) (90-100%) and granulomatosis with polyangiitis (GPA) (50-80%). The classic clinical presentation includes acute kidney injury with hematuria and proteinuria, accompanied by ANCA positivity (MPO-ANCA or PR3-ANCA). Histologically, the pre

Association Between the Earliest CALLY Index at Diagnosis and Progression to End-Stage Kidney Disease in Patients with Microscopic Polyangiitis and Granulomatosis with Polyangiitis.

Background and Objectives: This study aimed to determine whether the earliest C-reactive protein (CRP)-albumin-lymphocyte (CALLY) index at diagnosis was associated with progression to end-stage kidney disease (ESKD) during follow-up in patients with microscopic polyangiitis (MPA) and granulomatosis with polyangiitis (GPA). Materials and Methods: This study included 247 patients with MPA and GPA. The CALLY index was calculated as follows: serum albumin (g/L) × lymphocyte count (/mm3)/CRP (mg/L)/10,000. The earliest CALLY index was defined as the value calculated at diagnosis. ESKD, which occurr

Research progress on the diagnostic value of ANCA in autoimmune diseases of patients with refractory rhinosinusitis.

Anti-neutrophil cytoplasmic antibody (ANCA) is an important serological marker for autoimmune diseases, particularly ANCA-associated vasculitis (AAV). AAV frequently presents with refractory rhinosinusitis as an initial manifestation, yet ANCA testing in patients with rhinosinusitis continues to face challenges, including inconsistent subtype classification, variable detection performance, and difficulties in early identification, leading to misdiagnosis and delayed treatment. This narrative review focuses on the diagnostic value of ANCA in AAV-related rhinosinusitis, encompassing granulomatos
