Risk of Coronary Obstruction in Redo-Transcatheter Aortic Valve Replacement Between Bicuspid and Tricuspid Aortic Valves.

BACKGROUND: Transcatheter aortic valve replacement (TAVR) is approved across all risk profiles, including patients with bicuspid aortic valves. These patients are generally younger, with a higher chance of reintervention. OBJECTIVES: The aim of this study was to compare the feasibility of redo transcatheter aortic valve (TAV) between bicuspid and tricuspid aortic valves. METHODS: A computed tomographic (CT) simulation of redo-TAV was conducted using 913 post-TAVR CT studies from patients who underwent TAVR with SAPIEN 3 or SAPIEN 3 Ultra (S3; n = 623) or Evolut R, Evolut PRO, or Evolut PRO+

Ebstein Repair in a High-Altitude Setting ≥2,500 m: First Experience from Bolivia.

Contemporary surgical approaches for Ebstein anomaly are based on a paradigm shift towards earlier surgery in order to avoid the deleterious effects of chronic right ventricular (RV) volume overload. In addition, RV dysfunction may worsen in the setting of high altitude, and to date, no results on Ebstein anomaly surgery have been reported from a high-altitude setting. We herein present first postoperative results from Ebstein anomaly patients who underwent cone reconstruction (with or without bidirectional Glenn anastomosis) in Cochabamba, Bolivia (>2,500 m above sea level) using a specific hi

Sex-based differences in patients with Loeys-Dietz syndrome: An analysis of arteriopathies and surgical interventions.

BACKGROUND: Loeys-Dietz syndrome (LDS) is a rare connective tissue disorder caused by variants in genes like TGFBR1, TGFBR2, TGFB2, TGFB3, and SMAD3, leading to vascular abnormalities such as aneurysms and dissections. This study examines sex-based differences in the clinical presentation of LDS, focusing on comorbidities, physical exam findings, vascular events, and surgeries. METHODS: A retrospective analysis was conducted on 94 LDS patients (47 males and 47 females) at Mayo Clinic locations in Arizona, Rochester, and Florida. Data were collected from chart reviews, including comorbidities,

Left ventricular rotational abnormalities in adult patients with congenital heart disease late after Fontan procedure: detailed analysis from the CSONGRAD Registry and MAGYAR-Path Study.

BACKGROUND: The Fontan procedure (FP) is a surgical palliation diverting blood flow from the caval veins to the pulmonary artery. The FP is used in several congenital heart diseases (CHDs), for instance in the absence of a heart valve and/or in the presence of an abnormality of a heart chamber. Since little information is available on the cardiac mechanics of FP-operated patients, the present study aimed to determine three-dimensional speckle tracking echocardiography (3DSTE)-derived basal and apical left ventricular (LV) rotations in adult patients with CHD mainly affecting the right heart la

A case of heart failure due to multiple late complications after repair of tetralogy of Fallot in adulthood.

UNLABELLED: The prognosis of tetralogy of Fallot (TOF) has improved in recent years, but complications in the late postoperative period remain a serious problem. These complications, combined with specific hemodynamic and structural abnormalities, make it difficult to determine the optimal treatment plan. A man in his early 60s had been diagnosed with TOF in his early teens; he had undergone ventricular septal defect closure and right ventricular outflow tract repair in his mid-20s. Approximately 40 years after surgery, he was referred to our hospital because of worsening heart failure due to

Fetal Conduction Disease and Arrhythmia in Ebstein's Anomaly and Tricuspid Valve Dysplasia Assessed by Fetal Magnetocardiography.

BACKGROUND: Ebstein's anomaly and tricuspid valve dysplasia are associated with perinatal mortality approaching 50% and are among the most difficult fetal conditions to manage. Prediction of perinatal mortality is difficult and remains an active area of research. The usefulness of electrophysiologic indicators has not been investigated. METHODS: Eighteen fetuses with a diagnosis of Ebstein's anomaly and tricuspid valve dysplasia were referred for fetal magnetocardiography evaluation. We assessed fetal rhythm, waveform intervals, and heart rate variability and reactivity. RESULTS: Two subject

Surgical Aortic Valve Replacement For Bicuspid And Tricuspid Valve Disease: 7-Year Outcomes In >1100 Patients.

INTRODUCTION: Bicuspid aortic valve affects 0.5-2% of the population in developed countries. Given uncertainties about the best aortic valve replacement (AVR) option in this often younger, low-risk, population, it is important to understand how newer bioprostheses perform in these patients. The primary objective of this analysis was to compare 7-year outcomes of surgical AVR (SAVR) with the Avalus bioprosthesis between patients with a congenital bicuspid or tricuspid valve. METHODS: This prospective, non-randomized study included 1132 patients with aortic valve stenosis or chronic severe aort

Genotype-Phenotype Correlations, Treatment, and Prognosis of Children With Early-Onset (Neonatal) Marfan Syndrome.

Early-onset Marfan syndrome (eoMFS) is a severe and rare form of Marfan syndrome characterized by severe atrioventricular valve insufficiency developing before or shortly after birth. It is unclear which factors (interventions and/or genotype) influence survival. Forty-one individuals with eoMFS with a fibrillin-1 gene (FBN1) variant in exon 24-32 (CRCh37) were included. At the last follow-up, 14/41 (34%) were alive (8 months-18 years) and 27/41 (66%) were deceased. Median age of death was 1 month and 88% of the deaths occurred before 5 months of age. More individuals alive past the age of 16

Automated detection of pediatric congenital heart disease from phonocardiograms using deep and handcrafted feature fusion.

Congenital heart disease (CHD) is the most common type of birth defect, impacting about 1% of live births worldwide. Echocardiography, the gold-standard diagnostic method, is costly and inaccessible in low-resource settings. Diagnosis is delayed due to limited skilled experts, whose ability to interpret pathological patterns varies significantly, causing inter- and intra-clinician variability. Therefore, we present a new method for a more accessible diagnostic modality, the digital stethoscope, to detect CHDs. Our method is based on deep feature fusion, integrating deep and handcrafted feature

Infective Endocarditis 19 Months After Transcatheter Ostium Secundum Atrial Septal Defect Closure in a 6-Year-Old.

BACKGROUND: Infective endocarditis (IE) after transcatheter atrial septal defect (ASD) closure is rare. CASE SUMMARY: We report the case of a 6-year-old girl who developed methicillin-resistant Staphylococcus aureus IE 19 months after ASD closure with an Amplatzer device. She presented with severe sepsis, a large vegetation on the right atrial side of the occluder, and moderate tricuspid regurgitation. Despite empirical antibiotic therapy, persistent bacteremia mandated surgical removal of the device, ASD closure with a pericardial patch, and tricuspid valve repair. Six weeks of targeted anti

Ebstein's anomaly: an electrophysiological perspective.

Ebstein's anomaly of the tricuspid valve (EA) is an uncommon congenital cardiac malformation. It can present with atrioventricular tachycardia (AVRT), atrioventricular nodal re-entrant tachycardia (AVNRT), atrial arrhythmias, and rarely with ventricular tachycardia. The 12-lead electrocardiogram (ECG) is critically important and often diagnostic even prior to an electrophysiology study (EPS). Due to its complex anatomy, it poses particular challenges for mapping and ablation, even for an experienced electrophysiologist. In this review, we aim to provide insight into the electrophysiological pe

Perventricular device closure of the ventricular septal defects in children-first experience in the United Arab Emirates.

BACKGROUND: Ventricular septal defect (VSD) is the most common congenital cardiac malformation, accounting for approximately 30% of congenital heart defects. Conventional surgical repair using cardiopulmonary bypass is invasive and associated with morbidities and prolonged hospital stay. With the advent of interventional approaches and availability of different occluding devices, the technique of perventricular device closure is evolving and being implemented successfully in larger groups of patients. We present herein, our initial experience of perventricular device closure for the ventricula

Cardiac decompression and right ventricular function improvement after bar removal in patients with pectus excavatum.

Pectus excavatum, the most frequent malformation of the chest wall, has been related to cardiac compression and exercise intolerance. Cardiac outcomes after minimally invasive repair of pectus excavatum with retrosternal implants, particularly after removal (> 2 years postoperative) are generally unknown. We evaluated stress echocardiography outcomes before repair and after bar removal. This study comprised a retrospective cohort of patients with diagnosis of isolated pectus excavatum who underwent stress echocardiography before minimally invasive repair with retrosternal implants, and after b
