Longitudinal Electronic Cigarette Exposures Impair Respiratory Function and Structure in the Female Apoe-/- Mouse.

INTRODUCTION: Electronic cigarette (EC) usage amongst naïve users in the United States has been rising for the last decade. The effects of inhaling e-cig aerosols on respiratory health are not fully understood. The goal of this study was to longitudinally track changes in inflammation alongside abnormalities in lung structure and function following 8, 16, and 24 weeks of EC exposure. AIMS AND METHODS: Female Apoe-/- mice were nose-only exposed to either EC aerosols (JUULTM, 3% nicotine, tobacco flavored, 2 puffs/min, particle concentration of 300 mg/m3) or room air for 95 minutes, 5 days/week

Bronchogenic cyst presenting as an esophageal mass.

The patient was a 36-year-old female. The patient underwent gastroscopy examination two months ago. Gastroscopy revealed a mass in the upper esophagus. Then the patient was admitted to our hospital for endoscopic treatment. After admission, the patient underwent endoscopic ultrasound. Endoscopic ultrasound revealed a hypoechoic lesion measuring 20.6 * 10.5 mm in the upper esophagus. The lesion originates from the intrinsic muscle layer. Approximately three-quarters of the lesions are located outside the cavity. The patient's chest enhanced CT showed cystic low-density lesions in the esophageal

Protective Role of Apelin in a Mouse Model of Post-Intensive Care Syndrome.

Post-intensive care syndrome (PICS) is a serious condition involving physical weakness, depression, and cognitive impairment that develop during or after an ICU stay, often resulting in long-term declines in quality of life. Patients with acute respiratory distress syndrome and severe coronavirus disease (COVID-19) are at particularly high risk, yet the molecular mechanisms underlying PICS remain poorly understood. Here, we identify impaired Apelin-APJ signaling as a potential contributor to PICS pathogenesis through the disruption of interorgan homeostasis. Using a mouse model combining acute

Changes in sputum viscoelastic properties and airway inflammation in primary ciliary dyskinesia are comparable to cystic fibrosis on elexacaftor/tezacaftor/ivacaftor therapy.

BACKGROUND: Primary ciliary dyskinesia (PCD) and cystic fibrosis (CF) are muco-obstructive lung diseases that are caused by distinct genetically determined defects in mucociliary clearance; however, knowledge on the relative severity of airway mucus dysfunction and chronic inflammation remains limited. The aim of this study was therefore to compare sputum viscoelastic properties, inflammation markers and the proteome between patients with PCD and patients with CF before and under elexacaftor/tezacaftor/ivacaftor (ETI) therapy. METHODS: We compared sputum rheology, inflammation markers and the

Impact of Seasonal Variability in Nasal Nitric Oxide Measurements for Primary Ciliary Dyskinesia Screening.

BACKGROUND: Nasal nitric oxide (nasal NO) measurement is the most widely used screening tool for primary ciliary dyskinesia (PCD), but its accuracy may be affected by various factors. Based on our clinical experience, we hypothesized that nasal NO values vary across seasons, being lower in winter. This study investigates seasonal nasal NO variability in subjects referred for PCD screening. METHODS: This retrospective study (2010-2023) analyzed 434 subjects with clinical symptoms suggestive of PCD who underwent nasal NO testing at Schneider Children's Medical Center in Israel. Of these, 105 co

Bronchopulmonary sequestration in an adult patient in his late 60s.

Bronchopulmonary sequestration (BPS) is a congenital anomaly of the lung characterised by non-functioning pulmonary tissue that receives its blood supply from an anomalous systemic artery, most commonly from the aorta. Although typically diagnosed in childhood or adolescence, BPS can present later in life and may mimic more common pulmonary conditions. We present the case of a male patient in his late 60s with a persistent non-productive cough, in whom imaging revealed a sequestered lung segment with a feeding vessel arising directly from the descending aorta. This case emphasises the importan

Activating KRAS mutations mark premalignant cystic structures in congenital pulmonary airway malformations.

BACKGROUND: Congenital Pulmonary Airway malformations (CPAM) may lead to malignant degeneration, and therefore many surgeons opt to resect CPAM even in asymptomatic patients. Previously, we identified Kirsten rat sarcoma virus (KRAS) mutations in a subset of CPAM patients, possibly indicating a pre-malignant state. In order to unify treatment strategy in (asymptomatic) patients we focused on KRAS mutations as a potential risk factor for developing malignancy in CPAM. METHODS: Resected lung tissue of CPAM patients was separated in affected region ("cyst") and non-affected region ("control") to

Sputum from Individuals with Primary Ciliary Dyskinesia Drives M2-like Macrophage Polarization.

INTRODUCTION: Primary ciliary dyskinesia (PCD) is a rare, congenital condition in which impaired ciliary function leads to bronchiectasis and progressive lung function decline. Bronchiectasis development is believed to involve infection and inflammation but is incompletely understood. Macrophages play a central role in cellular immune response, contributing to both pathogen clearance and immunoregulation. Depending on local stimuli, macrophages are polarized towards pro-inflammatory (M1) or pro-resolution/phagocytic (M2) phenotypes. This study aims to investigate the effects of PCD sputum on m

Impulse oscillometry and traditional pulmonary function testing: correlation, advances and clinical implications.

Impulse oscillometry system (IOS) is an effort-independent pulmonary function testing technique that assesses respiratory system mechanics during tidal breathing and has gained increasing interest as a complementary modality to traditional pulmonary function tests (PFTs), while its physiological relevance and clinical role alongside conventional tests remain incompletely defined. This narrative literature review synthesises evidence on the correlations between IOS parameters and traditional PFT indices, summarises clinical applications across major chronic respiratory diseases, and discusses e

The Protective Effect and Molecular Mechanism of Tetrandrine on Male Reproductive Damage Caused by Silicon Dioxide.

The long-term inhalation of free silica dust causes silicosis-a prevalent occupational hazard-yet its systemic effect on male reproductive toxicity remains underexplored. Tetrandrine (Tet) is the only plant-derived anti-silicosis drug approved in China. This study investigates silica (SiO2) -induced male reproductive damage and evaluates Tet's protective effects. Sixty male C57BL/6 mice (6-8 weeks) were divided into control, SiO2 exposure, and SiO2 + Tet groups. SiO2 was administered via intranasal infusion and Tet via gavage. Mice were sacrificed at day 7 (male reproductive injury model corre

Tidal breathing nasal nitric oxide measurement as a test for primary ciliary dyskinesia in young children.

BACKGROUND: Nasal nitric oxide (nNO) measurement during velum closure is a useful test for primary ciliary dyskinesia (PCD) but is not feasible in young children. Measurement during tidal breathing is an alternative approach. Our objectives were to define a protocol with age-specific cut-off nNO values during tidal breathing and evaluate this method as an adjunctive test for PCD. METHODS: A standardised method for nNO measurement during tidal breathing was developed and tested at the University of North Carolina in children aged <6 years with PCD, asthma, cystic fibrosis and healthy controls.

Airway microbiome dysbiosis in severe pneumonia: metagenomic evidence of pathogen expansion and commensal depletion.

BACKGROUND: The pulmonary microbiome is increasingly recognized as a key determinant of pneumonia severity, yet its clinical implications remain incompletely understood. Disruption of microbial ecology, or dysbiosis, may impair host immune responses and exacerbate disease progression. This study aimed to characterize microbiome alterations associated with severe pneumonia and their correlation with host inflammatory and coagulative parameters. METHODS: In this multicenter, prospective observational cohort study conducted across nine hospitals in Shanghai (2021-2025), bronchoalveolar lavage fl

[Key updates in the 2025 ERS/ATS guideline for the diagnosis of primary ciliary dyskinesia].

In 2025, the European Respiratory Society (ERS) and the American Thoracic Society (ATS) jointly published a new guideline for the diagnosis of primary ciliary dyskinesia (PCD). Based on the Grading of Recommendations Assessment, Development and Evaluation (GRADE) methodology, this guideline provides a rigorous systematic evaluation of existing diagnostic tools, superseding the previous separate guidelines issued by the two societies in 2017 and 2018. This article aims to interpret the core content of the new guideline and compare it with previous guidelines, with the objective of providing a r

The safety of conservative management of asymptomatic congenital pulmonary airway malformations (CPAMs) in children: A systematic review.

PURPOSE: The optimal management of infants born with asymptomatic congenital pulmonary airway malformations (CPAMs) remains controversial. However, emerging evidence increasingly supports a conservative non-operative approach for these patients. We aimed to evaluate this evidence with the hope of clarifying optimal management for these patients. METHODS: A comprehensive search strategy was developed. Electronic databases MEDLINE and Embase Classic + Embase were searched from inception to December 7th, 2023. EBM Reviews and APA PsycInfo were searched from inception to November 30th, 2023. The

Declining frequency of surgical interventions over time in subglottic stenosis.

BACKGROUND: Soft tissue subglottic stenosis (SGS) progressively reduces the airway dimension below the vocal folds. The cause is unknown. As no cure is available patients face a recurrent need for surgical treatment. Previous research has not established whether the need for repeated interventions diminishes or intensifies as the disease evolves. Our hypothesis is that the number of surgical interventions decrease with disease duration. If true, these findings may have an impact on the surgical setting. AIMS/OBJECTIVES: To investigate if the number of surgical interventions decreases with dis
