Occlutech percutaneous patent foramen ovale closure: Safety and efficacy registry (OPPOSE).

INTRODUCTION: Safety and efficacy data on patent foramen ovale (PFO) closure with the Occlutech Figulla Flex II device are lacking. We undertook a fully monitored prospective Registry on PFO closure using this device. METHODS: 100 patients undergoing PFO closure were enrolled into the OPPOSE Registry at 6 UK centres. The primary endpoint was PFO closure (grade 0 or 1 shunt) at 6-month BCTTE assessed by Corelab. Secondary endpoints included implantation success, complications, and atrial fibrillation during follow-up. RESULTS: 100 patients aged 43.8±11.5years, 53% male, were recruited. Indica

Assessment of the MANTA closure device in transfemoral transcatheter aortic valve replacement: a single-centre observational study.

OBJECTIVES: The present study aims to evaluate the efficacy and safety of the MANTA vascular closure device (VCD) (Teleflex, Morrisville, NC, USA) in transfemoral transcatheter aortic valve replacement (TF-TAVR). BACKGROUND: To close the femoral artery in TF-TAVR a VCD is the treatment of choice. Data involving suture-based VCDs have been extensive. Although scarce, results on the MANTA device are promising. There is no consensus yet as to whether the MANTA device is associated with fewer access-site-related vascular/bleeding complications when compared to suture-based VCDs. METHODS: In this

Transthoracic device closure of perimembranous ventricular septal defect via a small left intercostal incision in children.

BACKGROUND: In children with perimembranous ventricular septal defect, surgical repair requires sternotomy and leaves unsightly scars, which can trigger long-term physical and psychological distress. However, transcatheter device closure is limited by vascular diameter, radiographic exposure, and expensive DSA equipment. We used an ultra-small left intercostal incision for transthoracic device closure to avoid the above problems and investigated its safety and feasibility by comparing it with surgical repair. METHODS: This study enrolled 358 children with perimembranous ventricular septal def

Use of the amplatzer vascular plug II device to occlude different types of patent ductus arteriosus in pediatric patients.

INTRODUCTION: Transcatheter patent ductus arteriosus (PDA) closure is difficult in small patients in part due to the diverse PDA morphology. We describe a multicenter pediatric experience using the Amplatzer Vascular Plug II device (AVP II) to occlude PDA. METHODS: All patients undergoing transcatheter closure of PDA with AVP II from April 2008 until May 2012 were included. Clinical, angiographic, and echocardiographic data were collected. RESULTS: One hundred and seventy-seven procedures were performed. Median age was 14 months (2-180) with median weight 9.7 kg (4.7-68). The morphological P

Minimal invasive aortic arch repair with suture-mediated closure system.

Port implantation can be associated with an array of serious vascular complications, typically involving the subclavian artery. We report a case in which implantation of a port resulted in iatrogenic perforation of the aortic arch at the level of the left subclavian artery, which was sealed off using a percutaneous vascular closure device.

Transjugular closure of a two-hole atrial septal defect in a child with iliac vein thrombosis.

The internal jugular vein is not a typical vascular access line during the percutaneous closure of an atrial septal defect. We report the closure of a double atrial septal defect with a single device, using a transjugular venous approach, in a child with an inferior vena cava obstructed by a thrombosis due to previous cardiac catheterization. That the transjugular venous approach can be used as a possible alternative during the transcatheter closure of an atrial septal defect in children, when the inferior vena cava access is not possible.

Tortuous Coronary Artery Fistula Closed Using a Microvascular Plug in Heart Failure.

BACKGROUND: Large coronary artery fistulas may result in ischemia and heart failure. CASE SUMMARY: A 74-year-old woman was found to have an ejection fraction of 20%-25% with apical and anterior akinesis. A large fistula from the left anterior descending artery to the pulmonary artery was identified as the likely etiology. Multiple attempts at closure failed in the setting of extreme tortuosity with nearly 180° bends. Eventually, a 5-mm vascular plug was advanced through a microcatheter and successfully deployed. Follow-up echocardiography demonstrated improvement of the ejection fraction to 4

Pulmonary blood flow alters nitric oxide production in patients undergoing device closure of atrial septal defects.

OBJECTIVE: To determine the effect of pulmonary blood flow (Qp) on nitric oxide (NO) production in patients with increased Qp due to an atrial septal defect (ASD). BACKGROUND: Alterations in pulmonary vascular NO production have been implicated in the development of pulmonary hypertension secondary to increased Qp. In vitro, acute changes in flow or shear stress alter NO production. However, the effect of Qp on lung NO production in vivo is unclear. METHODS: Nineteen patients (2.4-61 years of age, median 17) with secundum ASD undergoing device closure were studied. Before, and 30 min after A

Obturator foramen bypass for infective groin caused by closure device.

A 69-year-old man underwent carotid artery stenting through the right femoral artery with a percutaneous vascular closure device for hemostasis. Eleven days later, an infective femoral artery pseudoaneurysm was diagnosed by computed tomography. At surgery, a defect in the femoral artery was observed, corresponding to the remnants of the closure device. Removal of the foreign material, debridement, wound irrigation, and arterial patch plasty were performed, but the infection and leg ischemia did not improve. After several failed attempts to revascularize and control the infection, obturator for

Jugular Vascular Closure and Scar Formation after Leadless Pacemaker Implantation.

BACKGROUND: Achieving hemostasis of large bore venous access sites can be challenging and time consuming. Closure devices have proven to be superior in achieving hemostasis, reducing time to ambulation and improving patient comfort, compared to manual hemostasis techniques after femoral venous and arterial access. The closure of the jugular vein following large bore access has not been investigated in previous studies. In addition, scar formation of the neck after large bore access of the jugular vein has not been investigated. In this study, the safety and feasibility of the double Perclose P

Immediate Leg Mobilization is Feasible After Catheter Ablation of Atrial Fibrillation Using Large Vascular Access Sheaths (Pulsed Field and Cryoballoon Ablation).

BACKGROUND: Vascular recovery from catheter ablation (CA) has traditionally required a period of leg immobilization which can lead to discomfort and prolonged time to discharge. OBJECTIVE: The objective of this study was to compare a strategy of immediate leg mobilization (IM) using suture-mediated closure devices against traditional vascular recovery consisting of figure-of-eight suture and 4 h bed rest (BR) after CA of atrial fibrillation (AF) using large vascular access sheaths for cryoballoon ablation (CBA) and pulsed field ablation (PFA). METHODS: Two hundred subjects were retrospective

Percutaneous arterial closure for inadvertent cannulation of the subclavian artery--a call for caution.

An uncommon occurrence during central venous catheterization, inadvertent arterial sheath placement can cause potentially serious complications. When the subclavian artery is inadvertently cannulated, catheter removal may be complicated by significant hemorrhage due to its noncompressible location. We report a case of inadvertent insertion of a 7 Fr central venous catheter into the subclavian artery of a patient with severe kyphoscoliosis, Duchenne muscular dystrophy and cardiomyopathy. The catheter was successfully removed, however the initial attempt at closure device deployment resulted in
