INTRODUCTION: Deep Brain Stimulation (DBS) is an effective treatment extended broadly to many neurological and psychiatric disorders. Nevertheless, complications may arise during DBS procedures or following implantation due to implanted hardware. This may result in both minor and major adverse events that may necessitate hardware removal and/or compromise maximal therapeutic benefit for the patient.
OBJECTIVES AND METHODS: To identify relevant literature on hardware-related complications from DBS procedures by performing a systematic review, and propose how to identify at-risk group and possi…

BACKGROUND: Pelvic mesh implants are commonly used in female urology for the treatment of stress urinary incontinence and pelvic organ prolapse. However complications can be substantial and a number of the patients in this cohort proceed to mesh explantation. We evaluated urinary morbidity after pelvic mesh removal within New Zealand's National Mesh Service.
METHODS: Retrospective review of women undergoing pelvic mesh removal between March 2009 and December 2024 by the National Mesh Service. Primary outcomes were patient-reported urinary symptoms using validated instruments: Pelvic Floor Dis

BACKGROUND: The use of plate fixation to treat displaced midshaft clavicular fractures in adults reduces complications and residual shoulder disability. New features of the precontoured locking plates have been shown to reduce the need for hardware removal in adults. There is a lack of studies evaluating surgical fixation of displaced clavicular fractures with precontoured plates in adolescents. We evaluate outcomes and complications of adolescents with displaced midshaft clavicular fractures treated with precontoured locking plates.
MATERIALS AND METHODS: 40 adolescents with displaced midsha…

STUDY DESIGN: This is a case report.
OBJECTIVE: To report a case of soft-tissue reaction to wear debris and osteolysis around a pedicle scr … oft-tissue reactivity and inflammation surrounding the right T5 transpedicle screw.
RESULTS: After hardware removal, the patient's recovery was uneventful. Six months later, a repeated computed tomographic scan demonstrated resolution of the previously described soft-tissue mass and a satisfactory fusion of the thoracic spine.
CONCLUSION: Metal wear debris can f …

BACKGROUND: Hardware-related infection after deep brain stimulation (DBS) is one of the most serious complications and may need additional i … d.
RESULTS: The overall infection rate was 5.9% (6 of 102 patients). Management consisted of total hardware removal followed by intravenous antibiotics. The IPG was at first kept in a solution, then rinsed with water and dried following sterilization with ethylene oxide gas at 38 °C for 18 h. When the treatment of the infection was finished, we reused the IPG and …

Over the last two decades, operative management of clavicle fractures has become more commonplace. Although traditionally treated with plating techniques, clavicle fractures have also been treated with intramedullary fixation (IMF). While IMF is less invasive, it is not without complications, including hardware migration, and related symptoms. This study presents a novel method of clavicle IMF using titanium elastic nails inserted through the posterolateral clavicle. The principal investigator's experience with the novel technique at a singular institution was reviewed to develop a comprehensi

The incidence of proximal humeral fractures increases. The locking plate osteosynthesis is a standard procedure to treat displaced proximal humeral fractures. In the mostly affected elderly patients with an osteoporotic bone structure, complication rate is still high. An implant removal is commonly required. But also younger patients often request for hardware removal. The open implant removal with a subacromial and subdeltoideal arthrolysis is an extensive secondary surgery for the patients. We present a new technique to remove a locking plate from the proximal humerus arthroscopically with a…

Patients with CP who had femoral implants placed at or before age 16, and had ≥2-year postsurgical follow-up were included. The addition of a lateral extra-articular tenodesis to anterior cruciate ligament reconstruction has been shown to reduce graft failure and improve rotational stability, but current fixation methods often require additional tunnels or hardware, with risks of tunnel convergence, irritation, and hardware removal. This technical paper describes a modified Lemaire lateral extra-articular tenodesis technique in which the iliotibial band flap is secured directly to the femoral suture button of the anterior cruciate ligament graft, thereby avoiding additional fixation devices. This technique minimize…

A 50-year-old woman who had undergone cervical spine fixation 6 years earlier presented with dysphagia, regurgitation of undigested food, halitosis, and weight loss. Operative examination demonstrated a hypopharyngeal diverticulum with spinal hardware visible in a defect in the mucosa. She underwent an open cervical approach to removal of the hardware. Endoscopic staple diverticulotomy as described by Scher and Richtsmeier was performed 8 weeks later in the ambulatory surgical setting. After a period of enteral feeding via a nasogastric tube in the initial postoperative period, she was able to

BACKGROUND: Anterior cervical discectomy and fusion (ACDF) is a commonly performed procedure for degenerative cervical spine disease. Rare complications of ACDF surgery include hardware failure, in the form of screw loosening and migration, or rod breakage. We present a case in which we removed a migrated screw lodged in the esophagus from a patient with a failed anterior cervical fusion.
OBJECTIVE: To present a surgical technique and considerations to remove a migrated screw.
METHODS: The previous ACDF incision was reopened and exposure was gained under the guidance of a head and neck surge

BACKGROUND: Osteomyelitis following orthopedic hardware removal is a rare but serious complication, particularly in pediatric patients. This case report describes a unique instance of osteomyelitis of the great trochanter following the removal of a femoral nail in a 14-year-old girl, highlighting diagnostic challenges and management strategies.
CASE PRESENTATION: A 14-year-old Mediterranean white girl presented with persistent pain and swelling at the site of a previously removed femoral nail, used to treat a prior femoral shaft fracture. Symptoms developed 2 weeks post-removal, accompanied b…

BACKGROUND: To review our 10-year experience treating posttraumatic sequelae of the elbow using a modified Outerbridge-Kashiwagi (O-K) procedure.
METHODS: Twenty-one patients with posttraumatic sequelae of the elbow treated using the technique were evaluated clinically using the Mayo Elbow Performance Score, range of motion testing, and pain level. We noted the presence of preoperative and postoperative ulnar nerve symptoms, complications, and reoperations. Open contracture release was selected to address either removal of hardware or ulnar nerve pathology.
RESULTS: At a mean of 39 months (r

Instability of the craniovertebral junction (CVJ) following odontoidectomy is relatively common. Traditionally, separate stage posterior atlantoaxial ± occipitocervical fusion is used for treatment. A transmucosal approach using a clean-contaminated route is associated with hypothetical risks of infectious complications. There is a paucity of information in the literature assessing the risk of surgical site infection (SSI) using the transmucosal approach for hardware placement. The authors conducted a literature search through PubMed identifying patients with pathology requiring transmucosal (

BACKGROUND: Resorbable plating in cranial reconstruction for craniosynostosis has fewer reported complications than rigid hardware. Few long-term outcome studies exist for pediatric patients treated with this technology for cranial vault reconstruction.
METHODS: A retrospective review was performed on pediatric patients undergoing cranial vault reconstruction for craniosynostosis by 3 surgeons over a 15-year period. MacroPore (Cytori Therapeutics, San Diego, CA) or Lactosorb (Walter Lorenz Surgical Inc, Jacksonville, FL), composed of polyglycolic and polylactic acids, was used for resorbable
