Successful treatment of familial idiopathic lipid storage myopathy with L-carnitine and modified lipid diet.

We describe the clinical presentation, course, pathologic findings, and biochemical abnormalities found in three adult siblings with an idiopathic lipid storage myopathy. The major presenting symptoms were weakness and cramping, which were profound in one patient, moderate in the second patient, and mild in the third. All three individuals exhibited true myotonic discharges on EMG, normal or mildly depressed muscle free carnitine levels, and borderline delayed ketosis (40 hours) with fasting. Muscle biopsies from all three showed neutral lipid storage. Polymorphonuclear leukocytes and macropha

Clinical features and new molecular findings in muscle phosphofructokinase deficiency (GSD type VII).

Muscle phosphofructokinase (PFKM) deficiency, a rare disorder of glycogen metabolism also known as glycogen storage disease type VII (GSDVII), is characterized by exercise intolerance, myalgias, cramps and episodic myoglobinuria associated with compensated hemolytic anaemia and hyperuricemia. We studied five patients with PFKM deficiency coming from different Italian regions. All probands showed exercise intolerance, hyperCKemia, cramps and myoglobinuria. One patient had a mild hypertrophic cardiomyopathy. Biochemical studies revealed residual PFK activity ranging from 1 to 5%. Molecular genet

[A case of myopathy associated with hypothyroidism and hyperprolactinaemia].

UNLABELLED: We report a case of a serious myopathy related to hypothyroidism with pituitary enlargement and hyperprolactinemia. CASE REPORT: Mr D.L 33 years old man suffering from myalgia, muscular weakness and cramps for six months. The laboratory check-up revealed a high serum creatin phosphokinase level, an autoimmune hypothyroidism, hyperprolactinemia and pituitary enlargement. The electromyogram was normal and the muscle biopsy showed no obvious inflammation. The outcome was favourable under L. thyroxin. DISCUSSION: The frequency of myopathy during hypothyroidism ranges from 30 to 80%.

Enhanced dorsal premotor-motor inhibition in cervical dystonia.

OBJECTIVE: This study aims to understand whether the enhanced dPMI, seen in writer's cramp patients previously, extends to other populations of focal dystonia patients (e.g. cervical dystonia) as an endophenotypic marker. METHODS: We studied 9 healthy subjects and 9 patients with CD. dPMI was tested by applying conditioning transcranial magnetic stimulation to the left dorsal premotor cortex and then a test pulse to the ipsilateral motor cortex at an interval of 6ms. We also looked at the duration of the cortical silent period (CSP)-a measure of cortical excitability. RESULTS: CD patients ha

Muscle phosphoglycerate mutase deficiency: a study of a family with metabolic myopathy.

Muscle phosphoglycerate mutase deficiency results in a myopathic condition characterised by repeated cramps, possible myoglobinuria and muscle pain. We present a family with muscle phosphoglycerate mutase (PGaM) deficiency, mild glycogen storage and some exertional myalgia. Investigations of muscle biopsy tissue showed a residual PGaM activity in muscle due to a small amount of brain isoenzyme. The enzyme was also measured in blood cells and a full investigation of muscle metabolism was also carried out. Electron microscopy revealed mitochondria with similar morphological features in two patie

[McArdle's disease in a 14-year-old girl with fatigability and raised muscle enzymes].

INTRODUCTION: McArdle's disease is a disorder of muscle energy metabolism caused by a deficit of muscle phosphorylase. The typical form presents with fatigability muscle cramps and pains triggered by physical exercise. Some cases have few symptoms. We report the case of a 14 year old girl diagnosed on finding a significantly raised CPK, studied following her complaint of fatigability. CLINICAL CASE: A 14 year old girl presented with a CPK of 1,243 UI/l (normal 10-32) which had been requested in view of her fatigability. She had never had cramps, muscle pains or dark urine. Stiff-man syndrome: report of a case.

A 40-year-old Taiwanese farmer developed progressive stiffness and pain in the lower back with intermittent cramps over a 3-year period. The cramps were extremely painful and the patient was sensitive to any sudden movement or external stimulation. His lower back was so stiff that the patient had great difficulty performing daily activities and walked with a rigid and straight back. The stiffness of the muscles subsided during normal sleep. Except for a left S1 radiculopathy, no other neurologic abnormalities were noted. Electromyography showed spontaneous continuous muscle fiber activity whic

Priapism after western black widow spider (Latrodectus hesperus) envenomation.

Priapism in children after a black widow spider bite is a rare phenomenon with only a few case reports noted in the literature. Black widow bites are commonly associated with pain, muscle cramping, hypertension, and tachycardia. Initial treatment includes pain control with opiate or opioid medications and benzodiazepines, with antivenom reserved for severe cases of envenomation manifested by uncontrolled pain or hypertension. Treatment with antivenom for priapism is not well described; however, it has been noted to resolve priapism in the few cases that have been reported. We present a case of…

[Dystonias: what the practitioner needs to know].

Dystonia is characterized by involuntary muscular contractions resulting in repetitive movements and/or postures, with co-contraction of antagonist muscle groups. the present overview describes the fundamentals of dystonic movement disorders, and provides information on their diagnosis and therapy, including in particular the not infrequent focal dystonias spasmodic torticollis, blepharospasm, and writer's cramp, and the generalized forms. For the practitioner it is of primary importance to be able to recognize these sometimes bizarre movement disorders as dystonias. In many cases, the correct

Phenotypic variability of the DYT1 mutation in German dystonia patients.

Primary dystonia is a clinically and genetically heterogeneous movement disorder characterized by sustained involuntary muscle contractions causing repetitive movements and/or abnormal postures. Recently, the gene locus (DYT1) and mutation responsible for a substantial number of cases suffering from early-onset primary dystonia was described. Here we report 2 German families and 1 sporadic patient with early-onset dystonia due to the DYT1 mutation in order to illustrate the variability of clinical manifestation within this molecularly defined entity. We demonstrate that writer's cramp or focal

Muscle weakness, pain, and fatigue impair daily function in chronic kidney disease: a cross-sectional analysis from the I-RACE study.

Muscle dysfunction symptoms such as weakness and cramps are common in people with chronic kidney disease (CKD), but their impact on activities of daily living (ADLs) is not well understood. This study explored the association between muscle symptoms and patient-reported ADL impact in individuals with and without CKD. This cross-sectional secondary analysis of the I-RACE study (ISRCTN11596292) included adults (≥18 years) across the CKD spectrum and adults without CKD. Participants completed a bespoke Muscle Symptoms Scale assessing muscle-related symptoms and their impact on ADLs (daily activit

Three cases of recurrent generalized muscle spasms in China.

Three cases of recurrent generalized muscle spasms were reported. All three cases are of Chinese descent and show some typical features of recurrent painful muscle cramps, alopecia, diarrhea and abnormalities of bone and endocrine systems. Recurrent generalized muscle spasms or Satoyoshi's disease is recognized as an entity of the disease not only in Japan but also in China. The result that one of the three cases is associated with myasthenia gravis might suggest an immunological abnormality as a mechanism of the disease.

[Muscle phosphorylase deficiency in childhood. A case report].

Myophosphorylase deficiency or McArdle's disease is rarely recognized in childhood. The onset is generally in adolescence or in adult age with exercise intolerance, muscle cramps and myoglobinuria. Two siblings of 6 and 2 years of age are described. The first patient showed early fatigue and both had elevated CK levels. Morphological and biochemical studies of muscle biopsies revealed a defect of myophosphorylase.
