A retrospective study of cataract surgery performed between January 1983 and December 31, 1989, disclosed 48 eyes in which the posterior capsule was torn. The overall incidence of this complication was approximately 1% but was further reduced when the technique of capsulorhexis was mastered. Our review of the video tapes of these surgical procedures revealed that the tear occurred most frequently during nucleus removal (41%) and posterior capsular vacuuming (28%). The anatomy of each tear was classified and related to the event responsible for the torn capsule, as well as to the likelihood of

Intraocular lenses (IOLs) are implanted during cataract surgery. For optimum results, stable positioning of the IOL in the capsular bag is important. Wound-healing events following cataract surgery lead to modification of the capsular bag and secondary visual loss due to posterior capsule opacification. At present, it is unclear how these biological events can affect stability of the IOL within the capsular bag. In the present study, a human in vitro graded culture capsular bag model was the experimental system. Capsulorhexis and lens extraction performed on human donor eyes generated suspende

A 17-year-old girl presented with bilateral angle-closure glaucoma associated with spherophakia. A previous bilateral laser iridotomy failed to control intraocular pressure (IOP). Goniosynechialysis with lens aspiration and posterior chamber intraocular lens implantation were performed in both eyes. Peripheral iridoplasty was performed 3 days later. The postoperative IOP was controlled without medication for 12 months in the right eye and 24 months in the left eye. By restructuring the physiologic aqueous outflow route, goniosynechialysis safely and effectively treated secondary glaucoma from

A 64-year old man had noncontact holmium:YAG (Ho:YAG) laser thermal keratoplasty (LTK) performed in the left eye on March 10, 1998, and in the right eye on January 11, 1999. The patient achieved 1.3 diopters (D) and 1.4 D of corneal steepening in the right and left eye, respectively, which was the desired amount as his refractive error before Ho:YAG LTK was low. At the 3-month postoperative examination of the left eye, cortical cataracts were observed in both eyes. Approximately 1 year later, bilateral cataract extraction was recommended because of patient-reported decreased vision at distance

Alport syndrome is an inherited disorder of type IV collagen, a major constituent of basement membranes. Eighty-five percent of cases are transmitted through X-linked dominant inheritance, although autosomal dominant and autosomal recessive inheritance has also been reported. Clinical manifestations of Alport syndrome include progressive glomerulopathy, sensorineural deafness, anterior lenticonus, posterior corneal dystrophy, and abnormal retinal pigmentation. Anterior lenticonus may lead to loss of vision because of progressive myopia or cataract formation. We report 2 cases of unusual catara

We describe a complication following anterior neodymium: YAG laser capsulotomy in a case of capsular bag distension which was diagnosed 21 months after cataract extraction with phacoemulsification and in-the-bag lens implantation. An anterior neodymium: YAG capsulotomy was performed and immediately after this the posterior capsule collapsed and wrinkled, causing a marked decrease in visual acuity that necessitated posterior neodymium: YAG capsulotomy.

Deep-set eyes present a technical challenge during phacoemulsification cataract surgery because restricted access to the superior globe limits instrument maneuverability, particularly when surgery is performed through a superior (head-end) approach. Although the temporal approach is commonly preferred in such cases, a superior approach may be indicated in selected situations based on corneal pathology, astigmatism, or surgeon preference. We describe a simple "downlook technique" to facilitate superior (head-end) phacoemulsification in a 67-year-old man with bilateral Grade III-IV nuclear catar

Resolving spatial protein dynamics in native human epithelial tissues presents a significant technical challenge, particularly in inherently curved or unevenly mounted specimens. Here, we introduce an image processing pipeline for high-resolution, compartment-based analysis of protein localization, using the native three-dimensional architecture of the human anterior lens epithelium and capsule complex as a robust ex vivo proof-of-principle platform for precise cell segmentation and quantitative analysis. This platform integrates whole-mount immunostaining, 3D confocal imaging, computational t

The three-dimensional heads-up display system (3D HUDS) is increasingly utilized by ophthalmologists and suggested to offer ergonomic benefits compared to conventional operating microscopes. We aimed to quantitatively assess the surgeon's neck angle and musculoskeletal discomfort during cataract surgery using commercially available 3D HUDS and conventional microscope. In this single-center comparative observational study, the surgeon conducted routine phacoemulsification surgeries using Artevo® 800 and Opmi Lumera® 700 (both from Carl Zeiss Meditec, Jena, Germany). The surgeon's intraoperative
