Good's syndrome (GS) is a rare, adult-onset combined B cell and T cell immunodeficiency with an associated thymoma. These patients have an increased risk of bacterial, fungal, viral and opportunistic infections. This report describes a 75-year-old female patient who presented with a full body rash and an anterior mediastinal mass. She underwent a biopsy of her rash and mass, which revealed erythema multiforme and WHO Type A thymoma, respectively. Ecthyma contagiosum, or Orf, is a specific infection of small ruminants accidentally reaching humans. Few cases have been reported in the literature to date. We report here a case of ecthyma contagiosum occurred in a 9-year-old girl after contact with a flock of ecthyma sheep. This Orf was then complicated by superinfection and then by erythema multiforme. Orf is often underdiagnosed, probably because of small ruminant breeders' knowledge of this disease and its benignity in humans. It is caused by a very resistant Parapoxvirus in the environment. Erythema multiforme is mainly caused by drug allergy and infections. This is the case of a HIV-positive, 49-year-old male, recently cured for syphilis, that presented erythema multiforme minor, five days after taking sildenafil. He had a fast recovery, only with the use of antihistamines. Cell-mediated allergy to sildenafil was confirmed six months later, with the use of patch-tests.

The purpose of this retrospective clinicopathologic study was to investigate the presence of eosinophils in oral and skin lesions of erythema multiforme. Seventeen cases were selected which fulfilled clinical and histopathologic criteria for the disease. Twelve out of 13 sections taken from skin biopsies and 4 out of 5 sections taken from oral lesions contained eosinophils in varying densities. Thirteen patients with oral erythema multiforme were assessed immunologically by skin testing and in vitro tests (lymphocyte transformation and rosettes). The results suggested a possible defect in cell-mediated immunity for most patients in this group.
