A titanium expandable pedicle screw improves initial pullout strength as compared with standard pedicle screws.

BACKGROUND CONTEXT: Pedicle screws are now standard for spinal arthrodesis as they provide three-column spinal stabilization. Decreased vertebral body bone density because of aging reduces the stability of the bone-screw interface, potentially increasing screw pullout or pseudarthrosis. Modifications to standard pedicle screw designs to improve screw stabilization may help to compensate for the detrimental effects of decreased vertebral bone density. PURPOSE: To evaluate differences in initial pullout strength of an expandable titanium pedicle screw as compared with a standard titanium pedicl

Temporal Changes in Reverse Torque of Locking-Head Screws Used in the Locking Plate in Segmental Tibial Defect in Goat Model.

The objective of this study was to evaluate changes in peak reverse torque (PRT) of the locking head screws that occur over time. A locking plate construct, consisting of an 8-hole locking plate and 8 locking screws, was used to stabilize a tibia segmental bone defect in a goat model. PRT was measured after periods of 3, 6, 9, and 12 months of ambulation. PRT for each screw was determined during plate removal. Statistical analysis revealed that after 6 months of loading, locking screws placed in position no. 4 had significantly less PRT as compared with screws placed in position no. 5 (p < 0.0

Weight-bearing recommendations after first metatarsophalangeal joint arthrodesis fixation: a biomechanical comparison.

BACKGROUND: This study sought to determine whether several metatarsophalangeal (MTP) fusion techniques require complete immobilization or if some level of weight-bearing could be recommended after surgery. A comparison of synthetic composite to actual bone was included in order to examine the validity of the testing conditions. METHODS: Four MTP fusion modalities were tested in synthetic composite bone models: unlocked plating, locked plating, crossed lag screws, and an unlocked plate with a single lag screw. Stiffness was calculated and then used to find the two most rigid constructs; the lo

MRI evaluation of resorbable poly lactic-co-glycolic acid (PLGA) screws used in pelvic osteotomies in children-a retrospective case series.

PURPOSE: The orthopaedic use of resorbable poly lactic-co-glycolic acid (PLGA) implants carries multiple potential benefits. To our knowledge, only one publication exists regarding the use of PLGA implants in pelvic osteotomies in children, and data regarding resorption and potential side effects are lacking for resorbable pelvic screws in children. The aim of this study is to present an MRI-based evaluation of the resorption pattern and local tissue reactions in a paediatric case series after pelvic osteotomies fixated with PLGA screws. METHODS: Twelve children who had undergone a Salter or

The effect of cement augmentation on pedicle screw fixation under various load cases : results from a combined experimental, micro-CT, and micro-finite element analysis.

AIMS: Anchorage of pedicle screw rod instrumentation in the elderly spine with poor bone quality remains challenging. Our study aims to evaluate how the screw bone anchorage is affected by screw design, bone quality, loading conditions, and cementing techniques. METHODS: Micro-finite element (µFE) models were created from micro-CT (μCT) scans of vertebrae implanted with two types of pedicle screws (L: Ennovate and R: S4). Simulations were conducted for a 10 mm radius region of interest (ROI) around each screw and for a full vertebra (FV) where different cementing scenarios were simulated arou

A curved compliant spinal bone anchor to enhance fixation strength.

Pedicle screws have long been established as the gold standard for spinal bone fixation. However, their fixation strength can be compromised in cases of low bone density, particularly in osteoporotic bone, due to the reliance on a micro-shape lock between the screw thread and the surrounding bone. To address this challenge, we propose augmenting conventional pedicles screws with a curved compliant anchor. This anchor integrates a curved super-elastic nitinol rod that is advanced through a canulated pedicle screw, forming a macro-shape lock within the vertebral body to aid the fixation strength

[Anterior plating of the cervical spine with the hollow screw-plate system of titanium].

A hollow screw-plate system made of titanium for osteosynthesis of the cervical spine is presented. This system is designed to achieve a stable fixation for fusion of the vertebra. The posterior corticalis of the vertebral body need not and indeed cannot be perforated by the screws. Loosening of the screws is not longer possible because of growth of bone into the lumen of the screw and direct attachment of bone to the surface of the screw.

Correlation analysis of BMD in different regions of the vertebrae determined by QCT and DXA on pedicle screw loosening.

Low bone mineral density (BMD) impairs the stability of the bone-screw interface, which leads to screw loosening after spinal instrumentation. Quantitative computed tomography (QCT) and dual-energy X-ray absorptiometry (DXA) were applied to measure BMD of screw trajectories and other regions of the vertebrae in this paper, and the aim was to analyze the best effective tool and BMD of the best appropriate vertebral site to predict pedicle screw loosening after lumbar fusion surgery. 186 patients who underwent lumbar interbody fusion and pedicle screws placement were analyzed retrospectively. Crossed-screws provide greater tarsometatarsal fusion stability compared to compression plates.

BACKGROUND: Hallux valgus is a common deformity that is often treated with a fusion of the first tarsometatarsal (TMT) joint. Crossed-screws are currently the accepted standard but advances in plate systems present opportunities for improved clinical outcomes; however, in vitro testing should be performed prior to clinical implementation. The purpose of this study was to determine whether a locking plate with surgeon-mediated compression provides similar fusion stability compared to crossed-screws and if bone density or joint size are related to construct success. METHODS: Ten matched-pair ca

Magnetic resonance imaging analysis of the bioabsorbable Milagro interference screw for graft fixation in anterior cruciate ligament reconstruction.

Ligament graft fixation with bioabsorbable interference screws is a standard procedure in cruciate ligament replacement. Previous screw designs may resorb incompletely, and can cause osteolysis and sterile cysts despite being implanted for several years. The aim of this study was to examine the in vivo degradation and biocompatibility of the new Milagro interference screw (Mitek, Norderstedt, Germany). The Milagro interference screw is made of 30% ss-TCP (TriCalcium phosphate) and 70% PLGA (Poly-lactic-co-glycolic acid). In the period between June 2005 and February 2006, 38 patients underwent

Long-term outcomes of coracoid bone block osteolysis: case report.

Latarjet procedure is reliable and reproductible procedure for treatment of anterior shoulder instability. Results are durable but complications can occur: neurological injury, non-union, infection, stiffness, osteolysis of coracoid bone block, recurrence of dislocation, osteoarthritis. We present the case of two patients with complication not described in the literature: osteolysis of the neck of the scapula around screws discovered at 8 and 20 years after initial surgery. These patients presented with previous pain which motivated consultation. Imaging showed coracoid bone block osteolysis,

Identification of screw spacing on pediatric hip locking plate in proximal femoral osteotomy.

This study describes a computational analysis technique for evaluating the effect of screw spacing and angle on the pediatric hip locking plate system in proximal femoral osteotomy in pediatric patients having DDH with an aberrant femoral head and femoral angle. Under static compressive load conditions, the stresses of the screw and bone were examined as the screw spacing and angle changed. The spacing and angle of various screws were specifically considered as variables in this study based on the pile mechanism studied in civil engineering. As with the group pile mechanism, the tighter the sc
