People with severe mental illness (SMI) experience significant health disparities, and their end-of-life care remains underdeveloped. Employing a co-design approach, this study engaged a range of stakeholders in Hong Kong, including people with SMI, their family members and professionals in mental health and palliative care, to identify service gaps, challenges and opportunities for improving end-of-life care for this population. Findings highlighted the suboptimal state of end-of-life care for people with SMI and the underlying challenges at individual, organisational and community levels, in

Approximately 70% of aged care residents are transferred to a hospital during the last few months of life. Many factors contribute to hospital transfer at the end-of-life; however, little is known about the role of advance care planning on hospital transfer decision-making. A scoping review using Arksey and O'Malley's framework and the Joanna Briggs Institute Preferred Reporting Items for Systematic Reviews and Meta-Analysis Extension for Scoping Reviews was conducted to reveal the available research on hospital transfers of aged care residents at the end-of-life and the role of advance care p

BACKGROUND: Solid organ transplant patients experience high morbidity and mortality before and after transplantation. International guidelines recommend integrating palliative care into the management of patients with advanced organ failure, including transplant candidates and recipients, as it supports advance care planning, enhances communication and improves symptom management. However, it remains unknown how and to what extent palliative care is actually provided to these patients. The present scoping review aimed to systematically summarize and compare existing evidence regarding the inte

CONTEXT: Despite being a high-income country with universal health insurance coverage, disparities persist for LGBTIQ + people's mental health, substance use, sexual health, and physical health in Switzerland. Less research attention and public health efforts have focused on their palliative and end-of-life care (PEOLC) needs. This scoping review's objective is to explore, describe, and map the evidence related to PEOLC contexts for LGBTIQ + people in Switzerland.
METHODS: We conducted a scoping review in accordance with the Joanna Briggs Institute (JBI) methodology for scoping reviews and ad

AIM: This study aims to analyse and evaluate the theory of peaceful end of life to determine its relevance and applicability in nursing research, education and practice, particularly in the areas of palliative care and end-of-life care.
METHODS: We conducted a theoretical-reflexive study using specific criteria for analysing and evaluating middle-range theories as proposed by Ann Whall (2005).
RESULTS: The theory clearly outlines the dying and death process, connecting key concepts and essential statements that support a peaceful end of life. It also implicitly incorporates elements of the n

BACKGROUND: Social homecare workers provide essential care to those living at home at the end of life. In the context of a service experiencing difficulties in attracting and retaining staff, we have limited knowledge about the training, support needs and experiences of this group.
AIM: To gain a timely understanding from the international literature of the experience, training and support needs of homecare workers providing end-of-life care.
METHODS: We conducted a rapid review and narrative synthesis using the recommendations of the Cochrane Rapid Reviews Methods Group. Building on a previ

BACKGROUND: Improving palliative and end-of-life care for people with dementia is a growing priority globally. This study aimed to integrate multiple perspectives on end-of-life care for people with dementia and carers, to identify clinically relevant areas for improvement.
METHODS: The mixed-methods study involved surveys, interviews, and workshops with two participant groups: healthcare professionals and carers (individuals who provided care and support to a family member or friend). Healthcare professionals were invited to complete an online adapted version of the Australian Commission on

OBJECTIVES: To narrate the canon of knowledge around symptom control at end of life for patients using, or having recently used, non-invasive advanced respiratory support (NARS) at end of life for respiratory failure.
METHODS: A systematic review forming a narrative synthesis from a wide range of sample papers from Medline, Embase, CINAHL, Emcare, Cochrane and OpenGrey databases. A secondary search of grey literature was also performed with hand searching reference lists and author citations. The review was undertaken using the ENTREQ checklist for quality.
RESULTS: In total, 22 studies were

Neonatal deaths can be categorized in 5 modes along the dimension of intervention and physiology. This classification can be helpful to analyze the choices that can be made in end-of-life care in the NICU. In the Netherlands, neonatal euthanasia became an optional 6th mode of death since publication and legalization of the Groningen Protocol. This paper summarizes the history, legal status and ethical justification of the Groningen Protocol, and describes end-of-life practice in the subsequent years. Since the implementation of the Groningen Protocol, the practice of neonatal euthanasia has al

An increasing recognition over the past five decades of the importance of patients' autonomy and the right to be able to choose to limit medical treatment at the end of life has led to the development of a number of documents related to advance care planning, including the advance directive, medical power of attorney, and portable orders for life-sustaining treatment (POLST). While these documents are important aspects of advance care planning, without having goals-of-care conversations, a specific plan, and necessary supports to achieve these goals, the documents alone offer a false promise.

Dyspnea is a common and highly distressing symptom in patients with advanced illnesses. Many patients continue to experience chronic dyspnea despite optimal management of underlying disease(s) and various non-pharmacologic interventions, necessitating the consideration of pharmacologic therapies for palliation of dyspnea. One commonly asked question by clinicians is whether benzodiazepines have a role in the palliation of dyspnea. In this "Controversies in Palliative Care" article, three groups of thought leaders independently answer this question. Specifically, each group provides a synopsis

PURPOSE OF REVIEW: Stroke is a devastating illness. Significant progress has been made in the prevention, acute treatment and rehabilitation of stroke. Yet many people still die from stroke. The role of palliative and end-of-life care for stroke has received considerably less attention. This review presents an overview of the current understanding of palliative and end-of-life care for people with stroke.
RECENT FINDINGS: The impact of stroke is wide-ranging and long lasting. It affects both patients and those who become their caregivers. Those who are dying from stroke experience a similar s

BACKGROUND: The legalization of assisted dying remains open for debate in the United Kingdom, where the Terminally Ill Adults (End of Life) Bill is currently under review. It is paramount to consider patient and clinician attitudes on assisted dying and understand how legalization of assisted dying may alter care delivery in the United Kingdom.
OBJECTIVES: The purpose of this clinical review is to facilitate healthcare professionals' understanding of the clinical implications for practice and how their attitudes and those of their patients may influence the future of care delivery in the pall

OBJECTIVE: To map existing knowledge on palliative extubation in adult patients admitted to intensive care units.
METHOD: Scoping review conducted according to JBI guidelines. Searches were conducted between October and December 2024 in seven databases and four repositories, in addition to a reverse search. The selection was independent, the extraction followed a structured protocol, and the report used PRISMA-ScR. The analysis of the results was based on the PAGER framework (Patterns, Advances, Gaps, Evidence for practice and Research recommendations).
RESULTS: The search identified 1,463 m

Intensive care unit (ICU) nurses are widely regarded as essential contributors to end-of-life (EOL) decision-making, given their continuous bedside presence, close engagement with patients and families, and central role in interprofessional communication. However, a persistent gap remains between the conceptual recognition of this role and nurses' actual participation in formal decision-making. This scoping review summarizes current evidence on the barriers and facilitators shaping ICU nurses' participation in EOL decision-making and on the present state of their involvement in clinical practi

Life in an intensive care unit (ICU) involves the successful use of life-sustaining treatment and patients dying. In intensive care medicine, allowing a patient to die often means discontinuing life-sustaining measures. Together with the severity of the illness, this has a significant impact on the course of the dying process. End of life treatment and support focus on alleviating symptoms. The interprofessional team's task in the ICU is to anticipate, plan and implement palliative measures. A family-centered approach to palliative care requires providing human resources for psychosocial suppo
