Temporal bone histopathologic and genetic studies in Mohr-Tranebjaerg syndrome (DFN-1).

OBJECTIVE: To describe the temporal bone histopathologic and genetic abnormalities in a case of Mohr-Tranebjaerg syndrome. BACKGROUND: Mohr-Tranebjaezrg syndrome (DFN-1) is an X-linked, recessive, syndromic hearing loss, characterized by postlingual sensorineural hearing loss with onset in childhood, followed in adult life by progressive dystonia, spasticity, dysphagia, and optic atrophy. The syndrome is caused by mutations in the DDP (deafness/dystonia peptide) gene, which are thought to result in mitochondrial dysfunction with subsequent neurodegeneration. The temporal bone pathologic chang

Goblet cell density in acute otitis media caused by Moraxella catarrhalis.

HYPOTHESIS AND BACKGROUND: Secretory otitis media is associated with a highly increased goblet cell density, confirming the secretory pathogenesis of this disease. Previous studies have shown that the middle ear goblet cell density, and thus the secretory capacity, are massively increased during experimental acute otitis media and at least 6 months thereafter, conceivably predisposing to the subsequent development of secretory otitis media. These studies used middle ear inoculation of either Streptococcus pneumoniae, nontypeable Haemophilus influenzae, or H. influenzae type b. The present stud

Vestibular schwannomas: correlations between magnetic resonance imaging and histopathologic appearance.

BACKGROUND: The indication for surgery of vestibular schwannomas (VS) remains controversial and depends on several factors. The ability to predict their patterns of growth would allow better surgical planning. This growth may depend on tumoral proliferation but also depends on dystrophic changes. OBJECTIVE: The aim of this study was to evaluate the role of magnetic resonance imaging (MRI) in predicting the evolution of VS. For this purpose, the authors attempted (1) to compare the MRI appearance of VS with its histopathologic features, (2) to correlate the MRI appearance of VS and its histopa

Is cochlear implantation possible after acoustic tumor removal?

HYPOTHESIS: This study aimed to assess the possibility of hearing restoration after acoustic tumor removal. BACKGROUND: Hearing restoration surgery may be indicated after surgery of bilateral acoustic tumors or surgery of acoustic tumor in the only hearing ear. The choice is between cochlear implantation and brainstem implantation. METHODS: From the temporal bone collection at the House Ear Institute, Los Angeles, the author histologically examined eight temporal bones from seven patients who had undergone acoustic tumor removal during their lifetime. Special emphasis was put on examining th

Internal auditory canal involvement of acoustic neuromas: surgical correlates to magnetic resonance imaging findings.

OBJECTIVE: Factors that play a role in the selection of surgical approach for acoustic neuromas include patient health and age, size of tumor, hearing status, and location of tumor in the internal auditory canal (IAC) and the cerebellopontine angle. Deep extension into the IAC makes hearing preservation extremely difficult when a retrosigmoid craniotomy is used, and the best approach is a middle fossa subtemporal route. Modern gadolinium-enhanced magnetic resonance imaging (MRI) can be inaccurate in identifying the presence of tumor laterally in the IAC. This may affect the selection of a surg

Perilymphatic and endolymphatic pressure in the guinea pig after distal dissection of the endolymphatic sac.

HYPOTHESIS: The study was designed to investigate whether endolymphatic pressure exceeds perilymphatic pressure in an endolymphatic hydrops model with a partially functioning endolymphatic sac. BACKGROUND: Previous investigations of perilymphatic and endolymphatic pressure measurements during endolymphatic hydrops were done in a classic endolymphatic hydrops model, with a surgically blocked endolymphatic duct and sac. This model, in contrast to the clinical situation of Meniere's patients, totally lacks the functional contribution of the endolymphatic sac. METHODS: In the guinea pig, a parti

Characteristics of tone burst-evoked myogenic potentials in the sternocleidomastoid muscles.

HYPOTHESIS: Optimum stimulus parameters for tone burst-evoked myogenic responses can be defined. These optimized responses will be similar to those evoked by clicks in the same subjects. BACKGROUND: Loud tones give rise to myogenic responses in the anterior neck muscles, similar to click-evoked potentials, and are likely to be saccular in origin. METHODS: Tone burst-evoked and click-evoked myogenic potentials were measured from the sternocleidomastoid muscles of 12 normal subjects (6 men, 6 women) during tonic activation. The effects of tone burst frequency and duration were systematically i

Diagnosis of endolymphatic hydrops in vivo with magnetic resonance imaging.

HYPOTHESIS: High-resolution magnetic resonance imaging (MRI) at 1.5 T preferentially enhances the perilymph over endolymph after administration of contrast with gadodiamide, which allows for differentiation of the membranous labyrinth. Furthermore, this imaging allows for the detection of endolymphatic hydrops. BACKGROUND: Endolymphatic hydrops is believed to be associated with a number of ear diseases, including Ménière's disease. Although the pathologic changes of overaccumulation of endolymph in the inner ear are obvious on postmortem histologic examination of the temporal bone, they have
