Pain and pain control in children.

Focused research on pain and pain control in children has developed primarily in the last 10 years and even now is woefully inadequate in relation to the magnitude of the problem. The available research, inferences from the adult literature, and anecdotal information all indicate the elusive nature of pain. Pain is not solely a fixed neurophysiologic response to a noxious stimulus but a product of the interaction of many variables such as age, cognitive set, personality, ethnic background, and emotional state. These factors exert a tremendous influence on the suffering which surrounds the pain

Minimally invasive surfactant therapy with a gastric tube is as effective as the intubation, surfactant, and extubation technique in preterm babies.

AIM: Preterm infants requiring surfactant replacement have been treated using the INSURE technique, which requires sedation and comprises tracheal intubation, surfactant instillation and extubation. However, minimally invasive surfactant therapy (MIST) does not require sedation, minimises airway injury and avoids placing positive pressure ventilation on an immature lung. This study compared the feasibility of the two techniques and the outcomes in preterm babies with respiratory distress syndrome (RDS). METHODS: Preterm infants with RDS prospectively received surfactant via a gastric tube pla

Multidrug intravenous sedation: determinants of the sedative dose of midazolam.

OBJECTIVES: The efficacy of multidrug intravenous sedation regimens in oral surgery is based on the ability to titrate opioids, benzodiazepines, and barbiturates to a desired level of relaxation. Dosage requirements to reach the initial sedation end points of slurred speech and ptosis of eyelids vary widely from one patient to another. STUDY DESIGN: An assessment of physical, cardiovascular, behavioral, and pharmacologic factors that might predict midazolam dosage requirements for the initial sedation titration was carried out with data collected from a large controlled clinical trial of fent

Cryogenic auriculotherapy reduces pain and opioid use in patients undergoing endoscopic carpal tunnel release surgery: a retrospective analysis.

BACKGROUND: Effective perioperative pain management for patients undergoing hand surgery is critical because of the risk of opioid misuse. Complementary approaches, including auriculotherapy (AT), for perioperative pain management have gained increasing attention. This study was designed to assess the analgesic properties of AT in patients undergoing endoscopic carpal tunnel surgery (CTR). MATERIALS AND METHODS: This retrospective study analyzed medical records of patients who underwent unilateral ambulatory CTR performed by a single surgeon under a regional block plus sedation, with or witho

Cannabis use and anaesthetic agent dosing in patients undergoing surgery or interventional procedures: a single-centre retrospective cohort study.

BACKGROUND: There is limited evidence that habitual cannabis consumption increases anaesthetic requirements. We hypothesised that cannabis use affects anaesthetic dosing among patients undergoing surgery or interventional procedures. METHODS: A total of 341 199 adult patients undergoing propofol sedation or volatile-based general anaesthesia at Beth Israel Deaconess Medical Center, Boston, USA, between 2008 and 2024 were included. The primary exposure was cannabis use, differentiated into medical or non-medical use (recreational use or cannabis use disorder). Primary outcomes were propofol do

Comparison of Cumulative Opioid Consumption Between Time-Programmed Decremental Infusion and Fixed-Rate Basal Infusion Modes of Intravenous Patient-Controlled Analgesia Following Mixed Surgery: Protocol for a Randomized Controlled Trial.

BACKGROUND: Effective postoperative pain management remains a clinical challenge, with a substantial proportion of patients experiencing moderate to severe pain after surgery. Intravenous patient-controlled analgesia (PCA) is widely used; however, conventional fixed-rate basal infusion does not account for the dynamic temporal pattern of postoperative pain, potentially leading to suboptimal analgesia and unnecessary opioid exposure. Time-programmed decremental infusion has been proposed as an alternative strategy to better align opioid delivery with postoperative pain trajectories, but high-qu

Antinociceptive Effects of α2/α3-Subtype-Selective GABAA Receptor Positive Allosteric Modulators KRM-II-81 and NS16085 in Male Rats: Behavioral Specificity.

Recent studies suggest that among the gamma-aminobutyric acid type A (GABAA)receptor subtype heterogeneity, α2/α3 subunits of GABAA receptors mediate pain processing. Therefore, α2/α3 subtype-selective GABAA receptor-positive allosteric modulators (PAMs) may be candidate analgesics. Antinociceptive effects of α2/α3 subtype-selective GABAA receptor PAMs have been reported, but the behavioral effects of these compounds have not been systematically evaluated. This study examined the behavioral effects of two α2/α3 subtype-selective GABAA receptor PAMs, KRM-II-81 and NS16085, in male rats. The ant

Anesthetic management of ventricular-peritoneal shunt implantation in osteogenesis imperfecta type IIB: A case report.

INTRODUCTION: Osteogenesis imperfecta (OI), a rare congenital disorder, has a risk of bone fracture and progressive bone deformity. OI type II is the most serious subtype, and very few reports on its anesthetic management exist. Patients face several anesthetic difficulties, of which easy fracturing of OI-affected bones is critical. Herein, we report our experience with the anesthetic management of a patient with OI type II. PATIENT CONCERNS AND DIAGNOSES: Through genetic testing, a 14-month-old girl (height: 45 cm, weight: 3.9 kg) was diagnosed with OI type IIB due to COL1A gene abnormality.

Intrafollicular oocyte transfer (IFOT): Potential feasibility in the ovine species.

Intra-follicular oocyte transfer (IFOT) is a promising and innovative technique for in vivo embryo production previously described for equines and bovines. The aim of this study was to assess the feasibility of IFOT in the ovine species. Two preliminary in vivo and in vitro trials were performed to test the optimal procedures and timing for IFOT. In the in vivo trial, follicular growth was monitored with transrectal ultrasonography in ten adult ewes to preliminarily determine the ovulation and ideal timing for IFOT. The in vitro trial assessed i) the optimal inner diameter of the injection nee

Point-of-care Ultrasound for Diagnosis of Abscess in Skin and Soft Tissue Infections.

BACKGROUND: Traditionally, emergency department (ED) physicians rely on their clinical examination to differentiate between cellulitis and abscess when evaluating skin and soft tissue infections (SSTI). Management of an abscess requires incision and drainage, whereas cellulitis generally requires a course of antibiotics. Misdiagnosis often results in unnecessary invasive procedures, sedations (for incision and drainage in pediatric patients), or a return ED visit for failed antibiotic therapy. OBJECTIVE: The objective was to describe the operating characteristics of point-of-care ultrasound (
