Emergency management of severe hyperkalemia: Guideline for best practice and opportunities for the future.

Hyperkalemia is a common electrolyte disorder, especially in chronic kidney disease, diabetes mellitus, or heart failure. Hyperkalemia can lead to potentially fatal cardiac dysrhythmias, and it is associated with increased mortality. Determining whether emergency therapy is warranted is largely based on subjective clinical judgment. The Investigator Network Initiative Cardiovascular and Renal Clinical Trialists (INI-CRCT) aimed to evaluate the current knowledge pertaining to the emergency treatment of hyperkalemia. The INI-CRCT developed a treatment algorithm reflecting expert opinion of best

The chest pain choice decision aid: a randomized trial.

BACKGROUND: Cardiac stress testing in patients at low risk for acute coronary syndrome is associated with increased false-positive test results, unnecessary downstream procedures, and increased cost. We judged it unlikely that patient preferences were driving the decision to obtain stress testing. METHODS AND RESULTS: The Chest Pain Choice trial was a prospective randomized evaluation involving 204 patients who were randomized to a decision aid or usual care and were followed for 30 days. The decision aid included a 100-person pictograph depicting the pretest probability of acute coronary syn

An AI-driven evidence ecosystem for cardiometabolic nutrition: bridging the gap from multiomics to patient empowerment.

Cardiometabolic diseases remain a primary global health challenge, with nutritional interventions serving as a critical pillar of management. However, the rapidly expanding volume of nutritional research, spanning high-dimensional multiomics, real-world continuous monitoring, and highly heterogeneous clinical trials, has created fragmented knowledge silos, leading to a profound translation lag between scientific discovery and clinical practice. To overcome this methodological bottleneck, we propose a conceptual framework for an "AI-Driven Evidence Ecosystem." This perspective outlines a four-p

Molecular Mechanisms Underlying Progression and Spontaneous Regression in Desmoid Tumors.

Desmoid-type fibromatosis, also known as desmoid tumor (DT), is a rare and locally aggressive soft tissue neoplasm driven by constitutive activation of the Wnt/β-catenin signaling pathway, most commonly through CTNNB1 or APC alterations. While some tumors progress and cause substantial morbidity, approximately 25-38% undergo spontaneous regression during active surveillance, which is currently recommended as the first-line management strategy. Although advances in next-generation sequencing and transcriptomic profiling have elucidated the molecular drivers of tumor progression, the molecular m

The Impact of Oxidative Stress Imbalance on Ovarian Function and Its Mechanisms.

Oxidative stress, defined as an imbalance between reactive oxygen species (ROS) production and antioxidant defenses, plays a critical role in female reproductive health, particularly in ovarian function. This review provides a comprehensive and updated synthesis of the impact of oxidative stress on ovarian physiological processes, including follicular development, oocyte quality, steroid hormone synthesis, luteal function, and ovarian aging. We delve into the underlying molecular mechanisms-mitochondrial dysfunction, endoplasmic reticulum stress, inflammatory signaling (NF-κB, NLRP3), and epig

Developing an Evidence-Based and Patient-Centred Care Concept to Address Deficits in German Primary Care of Venous Leg Ulcers: Results of an Expert Survey Within the ULCUS CRURIS CARE Project.

Venous leg ulcers (VLU) account for the majority of chronic wounds, with an estimated rise in prevalence due to demographic change. Care often does not comply with evidence, and patients remain passive and uninformed. To support general practice VLU care, the 'UlcusCrurisCare' (UCC) project developed a multimodal intervention comprising provider training, software-supported case management and standardised patient education. Experts from the medical community (physicians, nurses, association of medical assistants), health insurance and patient representatives provided their assessment of barri

Pesticide Use and Safety Behaviors Among Farmers in Rwanda's Eastern Province: Perspectives from Sector Officers on Drivers, Factors, and Gender Differences.

Growing pesticide use is linked to increased health risks for farmers across Africa, due to factors such as improper handling, insufficient knowledge, and lack of resources. To better understand these risk factors in Rwanda, where the majority of the population and most women are farmers, we held focus group discussions (FGDs) with 31 Sector Agricultural Officers (SAOs) and Sector Animal Resources Officers (SAROs) in five districts of Eastern Province. Among their views on this topic, we identified four core themes among the FGDs of (1) interconnected challenges, (2) shared exposure and health

Structural and Functional Changes in Biological Systems of Wastewater Treatment Plants Induced by Bicyclic Non-Steroidal Anti-Inflammatory Drugs-A Review.

The increasing presence of pharmaceutical compounds in aquatic environments poses a significant challenge for wastewater treatment systems worldwide. Among these emerging contaminants, bicyclic non-steroidal anti-inflammatory drugs (NSAIDs) are particularly concerning due to their high consumption, partial metabolism, and long-lasting persistence in wastewater. This review was prepared critically based on popular databases such as PubMed and the Google Scholar website, and using the modern Nested Knowledge platform. The bibliometric analysis was performed using the VosViewer program with the k

Plastinated sectional anatomy applied in CT and MRI studies of the body of the crab-eating fox (Cerdocyon thous).

The crab-eating fox (Cerdocyon thous) is a wild canid with a wide geographic distribution and a high propensity for roadkill in Brazil. Accurate diagnosis and appropriate clinical-surgical management require in-depth anatomical knowledge of the species, which is still limited for C. thous. This study aimed to correlate the sectional anatomy of C. thous through plastination, computed tomography (CT), and magnetic resonance imaging (MRI) following the anatomical principle of metamerism to build the vertebrate body. Four animals underwent CT and MRI scans, and the images were stored in DICOM form

Evidence-based precision in TCM: A systematic review and Meta-Analysis of data mining for syndrome-specific acupoint combinations in acute asthma exacerbations.

BACKGROUND: The quest for personalized management of acute asthma exacerbations continues. Traditional Chinese Medicine (TCM) offers a rich repository of empirical knowledge, but its pattern-driven recommendations lack systematic, data-driven validation. METHODS: We performed a systematic data mining analysis of TCM clinical literature on acupoint sticking therapy for acute asthma. Latent structure analysis and frequent itemset analysis were applied to 40 studies involving 2107 patients to objectively identify core syndrome patterns and their associated acupoint combinations. RESULTS: We obj

Angiogenic microRNAs in Systemic Sclerosis: Insights into Microvascular Dysfunction and Therapeutic Implications.

Systemic sclerosis (SSc) is a complex connective tissue disease that affects the skin and internal organs and is characterized by immune dysregulation, progressive fibrosis, and microvascular dysfunction. Chronic tissue ischemia, accompanied by impaired angiogenesis, leads to the gradual loss of small vessels, resulting in clinical complications, such as Raynaud's phenomenon, digital ulcers, pulmonary arterial hypertension, and renal crisis. Emerging evidence highlights the crucial regulatory role of microRNAs (miRNAs) in vascular homeostasis through the modulation of key signaling pathways an
