Contact lens induced peripheral ulcers during hydrogel contact lens wear.

PURPOSE: Ulcerative keratitis is the most serious adverse response associated with contact lens wear. However, some reports of microbial keratitis in the literature may refer to a less severe form of corneal ulceration, which we have termed contact lens induced peripheral ulcers (CLPU). METHODS: We present details of 11 cases of CLPU which occurred in clinical trials at our research unit. RESULTS: CLPU episodes are characterized by a single, small (< 2 mm diameter) circular lesion in the corneal mid-periphery or periphery, involving a focal loss of the full thickness of the epithelium associ

Rigid gas permeable contact lens changes in the aphakic infant.

PURPOSE: The purpose of this study was to describe and evaluate the frequent parameter changes required in aphakic infants fit with rigid gas permeable (RGP) contact lenses. METHODS: Twenty-five eyes from 18 patients with congenital cataracts were fit postoperatively with RGP lenses and followed for an average of 19 months. The original lens specifications were determined by keratometer readings, corneal diameter, and axial length or retinoscopy obtained under sedation just prior to or after cataract extraction surgery. The frequency of changes in contact lens power, base curve, and diameter

Delivery of trifluridine to human cornea and aqueous using collagen shields.

PURPOSE: We investigated the ability of collagen shields to deliver trifluridine (trifluorothymidine [TFT]) to human cornea and aqueous humor. METHODS: 24-hour porcine collagen shields were soaked in commercially prepared TFT (Viroptic). Patients undergoing penetrating keratoplasty wore a pre-soaked collagen shield for at least 30 minutes preoperatively. Control patients received drops of TFT only. Cornea and aqueous samples were obtained. TFT levels were measured using high performance liquid chromatography. RESULTS: Among the four patients with intact corneal epithelium, TFT was detected i

Retained Aspergillus-contaminated contact lens inducing conjunctival mass and keratoconjunctivitis in an immunocompetent patient.

PURPOSE: A 66-year-old woman presented with a 1-month history of prominent mucoid discharge and foreign body sensation in her left eye. METHODS: Ocular evaluation revealed a moderately severe superficial punctate keratitis involving the temporal half of the left cornea. The superior tarsal conjunctiva showed marked papillary reaction with an area of indentation temporally. A mass was present in the superior temporal aspect of the fornix, clinically resembling a pyogenic granuloma. At the posterior aspect of this mass and covered by mucoid material, was a soft contact lens. RESULTS: Upon remo

Alignment effects in videokeratography of keratoconus.

PURPOSE: To determine the effects of corneal apex decentration on videokeratography in subjects with keratoconus. We controlled corneal apex decentration by first locating the cone apex position and then shifting the fixation position by known amounts relative to the videokeratograph axis. METHODS: We evaluated the effect of keratoconus apex position on videokeratography by measuring 28 eyes (17 subjects) at 4 corneal positions: the vertex normal, apex, and 1 mm or 2 mm above the apex. RESULTS: When compared to the apex alignment values, the radius of 1 mm (mean difference: 0.27 mm; SD +/- 0

Cornea-contact lens interaction in the aquatic environment.

PURPOSE: A large population of ametropic scuba divers wear contact lenses. We discuss optics and corneal physiology, as well as the types of contact lenses that are appropriate for underwater activities. METHODS: We reviewed an extensive body of literature to formulate guidelines to aid the contact lens fitter in satisfying individual sport diver's needs. RESULTS: Optical factors such as image displacement and light wave-length shifts require that contact lenses for underwater use be suitably modified. Underwater images appear nearer and larger (requiring greater accommodation) and are made

The effect of high and low Dk/L soft contact lenses on the glycocalyx layer of the corneal epithelium and on the membrane associated receptors for lectins.

PURPOSE: Bacterial adherence or binding to the target cell is a prerequisite for the initial stage of most infections and seems to be mediated by lectin-like ligands on the bacterial surface and specific receptors on the target cell membrane. The purpose of this study was to establish whether contact lens wear under closed eye conditions changes the glycocalyx layer, whether it exposes more lectin receptors than eye closure without a contact lens, and whether wear of low oxygen transmissibility (Dk/L) contact lenses exposes more receptors than high Dk/L contact lenses. METHODS: The eyes of si

The influence of center of gravity and lens mass on rigid lens dynamics.

PURPOSE: Center of gravity and lens mass have both been shown to influence rigid lens centration and stability. The purpose of this study was to investigate the relative contributions of these two factors to rigid lens dynamics. METHODS: Eight subjects (four with high-riding lenses and four with low-riding lenses) participated in the study. Each subject was fit with 12 lenses-six designs in each of two materials. Center of gravity and lens mass were recorded in every case. For each lens, four lens dynamics variables were assessed over a five-blink cycle for both the vertical and the horizonta

Initial treatment of microbial keratitis.

PURPOSE: The common occurrence of failed medical treatment in microbial keratitis led us to investigate this phenomenon. METHODS: We retrospectively reviewed all ulcers that presented to our department for 24 consecutive months. We classified each ulcer as either a therapeutic success or failure based on a precise definition of the response to initial antibiotic selection. We then analyzed multiple factors including: antibiotic selection, ophthalmic disease, ulcer characteristics, and management, to determine their significance in the success or failure in treating microbial keratitis.
