Advances in tuberculosis treatment: novel regimens, new drug pipelines, host-directed therapies, and updated management guidelines.

Tuberculosis treatment has undergone its most profound transformation since the launch of the standardised DOTS strategy in 1994. Since 2020, a series of pivotal randomised trials, including TB-PRACTECAL, ZeNix, Nix-TB, endTB, BEAT-TB, SHINE, and Study 31/A5349, have redefined the management of both drug-susceptible and drug-resistant tuberculosis. These studies have enabled shorter, fully oral regimens with improved efficacy and safety across adult and paediatric populations, including people with HIV, and have driven major updates to WHO treatment guidelines. Despite these advances, tubercul

Experimental Evolution of Drug Resistance for Antibiotic Target Discovery in Chlamydia trachomatis.

The recent application of experimental and computational drug discovery workflows has identified hundreds of new molecules that display potent, and in some cases highly selective, activity against bacteria of the genus Chlamydia. The full value of these novel antichlamydial molecules, however, depends critically on our ability to determine their molecular targets and modes of action. A particularly powerful and widely applied strategy for identifying candidate targets involves selecting for bacterial mutants that have acquired resistance to a compound's inhibitory activity, followed by determi

Enhancing the Functionality of Human Bone Marrow-Derived Mesenchymal Stromal Cells Using Signaling Modulators.

Bone marrow-derived mesenchymal stem/stromal cells (BM-MSCs) possess an inherent capacity to support hematopoiesis in vitro. As a result, they are often used as feeder layers for the ex vivo expansion of hematopoietic stem cells (HSCs) or coinfused with HSCs to improve the success of clinical transplants. However, MSCs isolated from non-hematopoietic tissues such as adipose or dental tissue may not be as effective as BM-MSCs in supporting hematopoiesis. Additionally, BM-MSCs tend to lose their functional ability after extended in vitro culture, which is necessary for producing sufficient cell

Animal-Free Intestinal Epithelial 2D "Organoid-Sheet" Cultures Using Invasin.

The intestinal epithelium consists of a single layer of polarized cells that serves as a crucial barrier against pathogens while mediating food digestion and nutrient absorption. The differentiated villus cells, important for the intestine's function, originate from rapidly dividing Lgr5+ stem cells located in the intestinal crypts. These stem cells can be expanded in vitro and form three-dimensional (3D) "mini-organs" known as intestinal organoids, which closely phenocopy the intestinal epithelium. The development and maintenance of these 3D organoids depend on a defined growth factor cocktai

An Integrated Workflow for Culturing and AI-Based Profiling of Mouse Intestinal Epithelium.

Two-dimensional (2D) culture systems are powerful in vitro tools that have revolutionized the study of intestinal stem cell biology. These culture systems derived from mouse intestinal and colonic epithelia enable the modeling of tissue architecture, barrier function, and disease mechanisms under physiologically relevant conditions. This protocol details methods for isolating and maintaining mouse intestinal crypts as 2D monolayers. Monolayers are established via enzymatic dissociation and seeding onto ECM-functionalized hydrogels. This protocol also describes a deep learning pipeline that per

Intestinal Organoids Culture in Synthetic Hydrogels.

Derived from adult intestinal stem cells, organoids form intricate three-dimensional structures that closely mimic the cellular diversity and spatial organization of the human gut epithelium. Patient-derived organoids (PDOs) can recapitulate patient-specific tumor heterogeneity, making them highly valuable for colorectal cancer research and personalized medicine. However, their long-term in vitro culture still depends mainly on animal-derived materials (e.g., Matrigel) with undefined composition, batch variability, and limited tunability. To overcome these limitations, we introduced the use of

Comprehensive Isolation Strategy for Intestinal Epithelial and Fibroblast Cells from Healthy and Tumor Human Tissues.

Organoids and fibroblasts derived from patient tissue serve as physiologically meaningful in vitro models to investigate tissue biology, diseases, and treatment responses. In this study, we present a robust protocol for the simultaneous isolation and long-term culture of patient-derived organoids (PDOs) and fibroblasts from both healthy and colorectal cancer samples. The workflow uses enzymatic dissociation followed by efficient separation into epithelial and stromal cell fractions. Organoids are embedded in Matrigel or Matrigel-Collagen I mixtures to support three-dimensional growth, whereas

A Tissue Explant Model for Colon Cancer.

Colorectal cancer (CRC) remains a leading cause of cancer-related morbidity and mortality worldwide. Recent advancements in cancer research emphasize the importance of personalized medicine, necessitating robust preclinical models that closely mimic the tumor microenvironment. Colorectal cancer organoids have emerged as a pivotal tool in this domain, providing a platform that preserves the cellular heterogeneity and allows high-throughput drug testing. However, organoids do not recapitulate the complexity of the tumor ecosystem, its architecture, and stromal interactions characteristic of nati

Alkoxy Radical-Mediated Alkene Difunctionalization via Electrophotochemical LMCT Catalysis for the Synthesis of Functionalized Ethers.

Alkoxy radical-mediated hydroetherification offers an atom-economical approach for C-O bond construction. However, the difunctionalization of alkenes driven by alkoxy radicals remains elusive due to the inherent challenges associated with the generation and controlled reactivity of these reactive oxygen-centered radicals. Here we report a homogeneous molecular electrophotochemical LMCT catalysis strategy that enables the difunctionalization of enol derivatives, achieving efficient alkenyletherification to access a diverse array of functionalized ethers. This method features mild conditions, br

Ligand-Free Eu(OTf)3-Enabled Photoactivation for Strain-Release Photocycloadditions of Bicyclo[1.1.0]butanes with Substituted Alkenes.

Generating 1,3-diradicals from bicyclo[1.1.0]butanes typically requires precious-metal photocatalysts or sensitizers. We report that commercial, ligand-free Eu(OTf)3 coordinates N-phenylacylimidazole BCBs, enabling direct 420 nm absorption to access a BCB-derived diradical without external chromophores. This single platform enables strain-release [2σ+2π] and [2σ+3π] photocycloadditions with electron-poor and aryl alkenes and annulations with α-vinyl azides. Mechanistic studies, gram-scale synthesis, and a sonidegib bioisostere demonstrate broad utility for sp3-rich scaffold construction.

Gold(I)-Catalyzed Regioselective C≡C Bond Cleavage of o-Alkynyl Anilines with tert-Butyl Nitrite to Prepare 2-Aminobenzonitriles.

In this work, a variety of 2-aminobenzonitriles were prepared in moderate to good yields through a gold(I)-catalyzed regioselective C≡C bond cleavage of o-alkynyl anilines with tert-butyl nitrite (TBN). The mechanistic studies revealed that the reaction involved a gold(I)-catalyzed 5-endo-dig cyclization of the o-alkynyl anilines to indoles, TBN-mediated nitrosation of indoles, and gold(I)-catalyzed C-C bond cleavage over three steps. The present method features a broad substrate scope, good functional group compatibility, and efficient and highly regioselective C≡C bond cleavage, with the ami

Enantioselective (3 + 2) Cycloaddition of Oxazepines and Bicyclo[1.1.0]butanes Catalyzed by Chiral-at-Metal Rh(III) Complexes.

Bicyclo[n.1.1] skeletons are valuable bioisosteres for aromatic rings in drug discovery, yet efficient and versatile asymmetric synthesis of chiral azabicyclo[2.1.1]hexanes (aza-BCHs) remains challenging. To address the unexploited cycloaddition strategy involving oxazepines and bicyclo[1.1.0]butanes (BCBs), we develop a chiral-at-metal Rh(III)-catalyzed enantioselective (3 + 2) cycloaddition. This protocol affords diverse chiral aza-BCH derivatives in 50-96% yields with up to 99% ee under mild conditions, providing facile access to such privileged three-dimensional skeletons. Hydrogen Bonding-Mediated Regio- and Stereoselective Divergent Synthesis of C4-Substituted Pyridines via N-Boryl Pyridyl Radicals.

Herein, we leverage intramolecular hydrogen bonding to selectively regulate the radical cross-coupling reaction, enabling the precise synthesis of C4-substituted pyridines with excellent regio- and stereoselectivity. Moreover, this unexpected strategy could be strategically applied to divergently forge functionalized pyridines via two-step, one-pot protocols involving traceless C-H activation. Notably, a CFTR potentiator (UCCF-29) could be readily prepared in 67% overall yield through this chemistry, achieving a 36% increase in yield relative to those with current methods. Fluoroiodocarbene Addition to Bicyclobutanes: A Radical Handle for Quaternary Fluorinated Bicyclo[1.1.1]pentanes.

Fluorinated bicyclo[1.1.1]pentanes (F-BCPs) are valuable three-dimensional bioisosteres in drug discovery, yet methods for the direct construction of bridge-monofluorinated and quaternary F-BCPs remain scarce. Here, we report a fluoroiodocarbene addition to bicyclobutanes under mild conditions, providing 2-fluoro-2-iodobicyclo[1.1.1]pentanes. The C-I bond serves as a versatile radical handle, enabling synthesis of quaternary bridge-fluorinated BCPs, including Giese-type additions to activated alkenes and desulfonylative couplings. This strategy offers a practical, modular entry to bridge-fluor

Modular Synthesis of Aerobactin-Aztreonam Conjugates with Enhanced Activity against Hypervirulent Klebsiella pneumoniae.

A modular strategy enabled controlled mono- and difunctionalization of aerobactin and late-stage attachment of alkyne-bearing payloads by CuAAC. Application to aztreonam afforded the first aerobactin-antibiotic conjugates. Monovalent conjugate 3b showed superior activity against hypervirulent Klebsiella pneumoniae KPN234 (MIC = 0.031 μM), a 16-fold improvement over aztreonam. Competition and iron-dependent activity studies supported aerobactin-pathway-dependent delivery, establishing aerobactin as a viable antibacterial delivery scaffold.
